Medical Records Request

X-ray and MRI Imaging Included

Orthopedic Associates of Dutchess County (OADC) has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests. Please follow the instructions below to submit your request for medical records information, X-ray or MRI images. Please note, records will not be released without a request and authorization.

Submit a Request

To submit a request, please visit HealthMark’s Request Manager by clicking the button below. Once you have logged in, select “Submit Request” from the menu and fill in all required fields to authorize HealthMark directly. If you need to confirm your applicable OADC practice address, please visit our locations page. Your request will then be processed, and you will receive an email notification once it is complete and available for download. If you have any questions, please call HealthMark at (800) 659-4035.
