How do I set up my Portal account??

The easiest way to set up an account is by requesting an email invitation from us. (Please provide the correct email address to one of Our Patient Representatives then we will send you an invite.
Patients can also create their account without an invite . Please go to www.orthoadc.com click on the Portal Link. Then you should will be able to request a connection to Orthopedic Associates of Dutchess County during this process. We then will verify their account and connect your medical records.

What is my Invitation code?? My invitation code is not working

the last 4 digits of your SSN or your 4 digit date year of Birth. If the system wont accept either please contact us at 845-454-0120

Is it possible to get my child's information through the FollowMyHealth?

No, we do not have proxy Access for FMH accounts. The patient has to be 18 and older

Do you have an iPhone/iPod/iPad and Android app ?

Yes we do. You can download the FollowMyHealth® Universal health record App to view your information from your Device

How do I change my contact email address?

Please note that this may only be done from a web browser on a computer. Any issues please give us a call

What happens when I add something to my Portal ?

For any Demographics or insurance changes we do get an electronic notification and we will update the information in our system.

**When the patient add an item to their universal health record, it gets stored online only. If they would like to update or change any medical information patient must compose and send a message to their provider. Our medical records Team would review it and send it the appropriate Department**.

I forgot my Username or password

If you have forgotten your username: Due to security reasons, we cannot see or access this information. In that case, please contact us so we can verify the Login Method

I cannot see my information

If you have created your portal account, but do not see any health information or your doctor, you should check your connections and see if you are not connected to any healthcare providers. You can connect with OADC by adding the connection through the My Account menu.

Please check your connection status. If you have requested a connection with OADC and the status is pending please allow us 24hrs so we can match your account with our electronic medical records.
