YOUR FIRST VISIT

There are several things that you can do to make your first visit go smoothly:

• Arrive 15 minutes early for your first appointment so that you can fill out any paper work that you were not able to download.

• If your insurance requires a referral please bring it with you or arrange to have it faxed or sent to our office.

• Have all medical records such as x-rays, MRI's, or scans that are pertinent to the condition

• Bring your insurance cards to your appointment

• Complete the Patient History form and bring it to your appointment(click her to obtain the form on-line)
• Review and sign the Financial Policy and bring it to your appointment (Click here to obtain form)

• Review the Notice of Privacy Practices (click Here to review)

• Bring your payment which is due at the time of service
